How to Prepare

Send records

To ensure we’re fully prepared for your pet’s appointment, we ask that you request your pet’s records from your previous veterinary clinic and forward them to us at:

This helps us avoid delays and ensures your pet receives the most informed care possible.

Bring supplements

If your pet takes anything that may not appear in their medical record — joint supplements, calming chews, probiotics, flea/tick products, etc. — please bring them or take a photo of the label.

This helps us give accurate, safe medical recommendations.

Dogs: secure leash

A secure leash or harness is perfect. For collars, the safest fit is the two‑finger rule — you should be able to slide two fingers comfortably between the collar and your dog’s neck. This helps prevent accidental slipping, especially in busy areas like our parking lot.

Cats: secure carrier

A secure carrier helps them feel safe and calm. We also have a quiet cat‑only waiting area and can provide a special towel to cover the carrier to reduce stress and help your cat relax before their appointment.

Paperwork

We’ll text your Welcome Form to you when your appointment is scheduled. You can complete it on your phone, or if you prefer paper, just let us know — we’ll have it ready for you.
(If you’re filling out paper forms, arriving a few minutes early is helpful.)

New Client Form

We do not post our Welcome Form online. Instead, we text it directly to new clients when the appointment is made to protect your privacy and ensure the most up-to-date version is used.
Additional forms (such as medical history or release forms) can also be texted to you as needed.

Payment Information

We’ll always provide clear estimates and discuss treatment options with transparency and respect. If you’d like to explore financing options, just let us know — we’re happy to help you understand what may be available for your family.

Pet Insurance & Cost Planning

Pet insurance can be a helpful way to plan for your pet’s longterm medical needs — but it works very differently from human insurance. With pet insurance, you pay your bill at the time of service, and your insurance company reimburses you afterward based on your plan.
